According to this Digital Health ID Card Yojana. The person who wants to participate in the Ayushman Bharat Digital Mission has to create their IDs in the ABDM portal.

Then a Digital Health ID Card containing a 14 digit number is issued in favor of the person applying for the Digital Health ID Card Yojana.
